Take the time to carefully look over the entire application before trying to complete the application.
Try to complete each and all sections of the application if possible.
Never underestimate your ability to compete in this program, let the judges on various levels of competition determine how well you will perform in this program.
Try to determine the proper category where involvement in an activity or organization properly belongs.
Do not duplicate information in more than one place on an application, points can be awarded only once if including the same information in more than one section of the application.
Be sure to obtain all required signatures on the last page of the Application.
Each contestant may use one (1) additional sheet front and back if necessary to list additional information that is not included on the CYLA Application.
Please list the category above all information listed on the separate sheet of paper.
• I A Spiritual Activity Church Related- This can be any spiritual activity or ministry that is directly associated with the students church parish.
• I B Other Spiritual Activities- This can be spiritual activity that the student participates or an organization that the student participates in that is directly involved with his or her school or any spiritual activity or organization that is not associated with his or her church parish.
• II Scholastic Achievement- Be sure to list the GPA of all four years of high school. The majority of this sections involves school related organizations. It may not necessarily aid the student to list every single activity within each organization.

• III Civic or Community- This section is where a student may want to list
organizations that may more directly associated with the local community or
an activity that the student may have participated in that assisted the local
Community.
• IV Athletic Events- This can be any athletic event or sport that the student
participated in that is either school related (Track, swimming, tennis,
football, baseball basketball or any school related team or sport) or none
school related sports or activities (AAU Sport teams, non based school
Bowling Leagues, Archery Competitions and etc.)
• V A Social Activities School Related- This is a section that the student
could social activities that are school related (school prom, other school
Dances, attending school sporting events, participating in school fashion
show, school homecoming dance or court)
• V B Social Activities Non School Related- Community Beauty Pageant or
Contest, Attending an Concert, Attending a Community Play, attending a
Professional or College Sporting event

ONE COUNCIL-GENERALLY, THE COUNCIL OF THEIR
HOME CHURCH PARISH.
IF YOU SUBMIT YOUR APPLICATION TO MORE THAN ONE
COUNCIL, YOU WILL BE DISQUALIFIED.
